Privacy Policy

At Principle Mind, your privacy is important to us. This policy outlines how we collect, use, and protect your information when providing psychiatric care, including via telehealth, email, text, and in-person services.


Who We Are

Principle Mind is a psychiatric practice based in Oregon, offering integrative mental health care through medication management and therapy in person and via secure telehealth platforms.


What We Collect

  • Personal Information: Name, date of birth, email, phone number, address, emergency contact, insurance details, and billing information
  • Health Information (PHI): Medical history, psychiatric assessments, diagnoses, medications, treatment plans, and clinical notes
  • Technology Information: IP address, browser type, and website usage data (cookies are used only for functionality).


How We Use Your Information

Your information is used to:

  • Provide psychiatric care and treatment
  • Schedule and manage appointments
  • Process billing and insurance
  • Communicate with you regarding your care
  • Maintain clinical records and improve service quality


Telehealth

We use HIPAA-compliant, encrypted platforms for telehealth sessions. While we take strong security measures, we recommend using a private internet connection and secure device for added protection during virtual visits.


Email & Text Messaging

With your consent, we may use email or text messages for:

  • Appointment reminders
  • Administrative updates
  • Non-sensitive communication related to your care


Please note: These communications may not always be encrypted. You may opt out at any time by notifying us.


Information Sharing

We only share your information when:

  • You provide written authorization
  • It is necessary for your treatment or billing
  • It is required by law (such as in cases of mandated reporting or court orders)
  • We are working with HIPAA-compliant partners who assist in providing services
  • We never sell or rent your information.
  • We do not transfer consumer data to external organizations under any circumstances, including with consent.


Your Rights

You have the right to:

  • Request and access your health records
  • Ask for corrections to your records
  • Request limits on how your information is used
  • Ask for communications at alternate locations or by alternate means
  • Receive a list of disclosures
  • File a complaint if you believe your privacy rights have been violated\


Data Security

We use secure platforms, encrypted systems, limited access controls, and staff training to protect your information. If a data breach occurs, we will notify you as required by law.


Record Retention

We maintain patient records in accordance with state and federal law. In general, adult records are kept for at least 7 years after the last date of service, and records for minors are kept until at least age 21.


Medical Records Request Policy


To ensure the highest standards of privacy and compliance with federal and state regulations, particularly regarding sensitive psychiatric information, our clinic follows strict protocols for the release of medical records.


We do not accept third-party faxed requests, including those accompanied by patient-signed release forms that are not submitted through our verified channels. These requests cannot be reliably authenticated and therefore do not meet our security standards.


We only accept medical record requests through the following methods:

  • Direct request from the patient via our secure patient portal or electronic release of information (ROI) system.
  • Written request signed in ink and mailed directly to our office by the patient.
  • Valid legal subpoena submitted through appropriate legal channels.


This policy exists to protect the confidentiality and integrity of psychiatric information, which is subject to heightened legal and ethical safeguards. We appreciate your understanding and commitment to protecting patient privacy.


Data Sharing
• Customer data is not shared with 3rd parties for promotional or marketing purposes.
• Mobile opt-in and consent are never shared with anyone for any purpose. Any information sharing that may be mentioned elsewhere in this policy excludes mobile opt-in data.


Messaging Terms and Conditions
• By providing your phone number and agreeing to receive texts, you consent to receive text messages from Principle Mind, from 503-404-4555 regarding . Consent is not a condition of purchase. Message frequency varies. Message & data rates may apply. You can reply STOP or Cancel to unsubscribe at any time or HELP for assistance. You can also contact us at 503-404-4555 or admin@principlemind.com. Mobile opt-in information is never shared with third parties.


Text will be informational only. No marketing text will be sent. 

  • Data Collection: We will collect your name, email address, mailing address, and mobile phone number when you sign up for SMS updates.  The information will be collected via text initiated from client with key word START. 
  • Opt-In: You can opt in to the SMS service at anytime by texting START to 503-404-4555 Message frequency varies. Message and data rates may apply. Text HELP for help and Text STOP or CANCEL to unsubscribe at any time.  
  • Data Usage: We use your data solely for sending replies to inquires. 
  • Data Security: We protect your data with secure storage measures to prevent unauthorized access. Our team is fully trained in data security protocols to protect your information and prevent security breaches.
  • Data Retention: We retain your information as long as you are subscribed to our SMS service. You may request deletion at any time. 
  • MESSAGE AND DATA RATES MAY APPLY. Your mobile carrier may charge fees for sending or receiving text messages, especially if you do not have an unlimited texting or data plan.
  • Messages are recurring, and message frequency varies.
  • Contact Principle Mind at 503-404-4555 or admin@principlemind.com for HELP or to STOP/CANCEL receiving messages.
  • Opt-Out: You can opt out of the SMS list at anytime by texting, emailing, or replying STOP or CANCEL to  admin@principlemind.com  or 503-404-4555 .  After unsubscribing, you will receive a final SMS to confirm you have unsubscribed and we will remove your number from our list within 24 hours.
  • You can send HELP for additional assistance, and you will receive a text including our Phone number, email and website. We are here to help you.
  • Non-Sharing Clause: We do not share your data with third parties for marketing purposes. Principle Mind  will not sell, rent, or share the collected mobile numbers.

We may update this privacy policy from time to time. The most current version will always be available on our website and upon request.
