Fees and payment 

Actual rates depend on your insurance, as we are contracted to provide services at a rate determined by your insruance.  We can not discount the rate charged to your insurance. 

If you choose not to use your insurance, our private pay rates are listed below.

Private pay/cash rates

 I am not able to take medicaid/medicare at this time. Your insurance will be billed and  any deductible or co payment amounts due will be charged to your card on file. 

• Consultations/evaluations are $500 for 60mins.  
• Subsequent follow up appointments for medication and therapy  at $275 per 30mins.

I also accept private pay, providing further discretion. For private pay services, payment is due at time of service.

• Consultations/evaluations are $600 for 90mins.  
• Subsequent follow up appointments for medication and therapy  at $275 per 30mins.

• Evaluations 60min  $400
• Follow up appointments 30min  $200
  

Cancellation policy is as follows: Please give a 48 hours notice. Cancellations must be managed online using your email confirmation. You can reschedule or cancel your appointments online using the booking link. You will be sent a verification code to your email on file. 

No-shows or missed appointments without 24 hour notice: $250 fee before you will be allowed to reschedule. Exceptions for true emergencies (family and medical emergencies).

This is not meant to be punitive,  with less than 24 hour notice, the appointment time is not able to be filled by another patient and 24 hours before your appointment, time is spent preparing for your appointment. By scheduling an appointment you are consenting to this policy.

Late Appointments: After 10 minutes, appointments will be cancelled, or in case of an emergency, appointment time will be reduced without refund. 

Our prescribing practices include using the minimal effective dose of medications, reducing  the number of medications one takes and limiting the use of controlled substances.  we aim to use minimal amounts of medication, for the shortest amount of time possible, with a focus on safely discontinuing and tapering medications when able.

Refills are requested online. This allows us to know that you are taking your medication, not having side effects, and are doing well. If you have contacted your pharmacy, and have no refills before your next appointment, request them using the link below. 

DO NOT have your pharmacy request your medications, as I do not reposed to these request. 

For medication changes, please schedule an appointment.

Controlled substances

We only prescribe controlled substances if we are your only prescriber for these medications. You must be seen at least every every 30 days if dose adjustments are needed and 90 days once stable.

We limit use of controlled medications, specifically benzodiazepines. We will verify your history using medical and prescription records from the last 3 months. Without these records, you will be restarted on treatment based on your history, our clinical assessment and guidelines.

We also have a patient portal which is secure and allows you to manage appointments and invoices.  We will return current patients emails and phone calls within 48 business hours. All emails are HIPAA compliant. 

We will return phone calls for current patients with routine medication or scheduling questions under 5 mins are, as a courtesy, free. For calls greater than this, a rate of $75 for every 15 mins may be charged on a case by case basis.

We are unable to  provide the following services :

• Emotional Support Animal Letters 
• Case management
• Disability Evaluations   
• Forensic Evaluations   
• Assessments for ability to return to work, ability to own a weapon, ability to drive a certain vehicle, etc.
• Court-ordered treatment, or Involuntary Treatment  

In the case of emergency, such as thoughts of harming yourself or others, please call 911 immediately or go to the nearest emergency room. You may also call the national suicide prevention lifeline at 1-800-273-TALK (8255), which is a confidential hotline staffed by trained mental health counselors 24 hours per day, 7 days per week.
