FOR CHANGES TO YOUR PREVIOUSLY SCHEDULED APPOINTMENT
To get started, you will need to fill out my new patient Inquiry form. You will be emailed scheduling instructions if your needs match the capacity of my clinic.
Initial appointments are one hour long and focuses on what is most important to you, the details of your history necessary for a diagnosis and the development of a treatment plan.
Follow up appointments are 30 to 60 minutes and occur at one month intervals while medications are being changed. Once stable, follow ups occur every 90 days.
More frequent and extended appointments, are available to established patients, as well as remote and in home visits.
A credit card on file will be required to book. You will not be charged if you cancel with at least a 24 hour notice, or emergencies requiring you to reschedule. Otherwise, missed appointments with less than 24 hour notice will be charged $250. Please be aware of the same day cancellation fee. This is not meant to be punitive, there is a lot of preparation that goes into your appointment and with less than 24 hour notice, the appointment time is not able to be filled by another patient.
